How do i get it street legal?

Headlight, taillight, left hand mirror, horn.

Fortunately, you should be able to do this in Ohio without too much trouble.  Take your title to the title office.  Tell them you want the affidavit to convert your title from off road to street bike.  After you've signed that they can issue you a title that indicates your bike is street legal.  Then, take your new title to get plates.  Note, the people at the title office may not be familiar with this process since so few people do it.  If they don't know what you are talking about, politely ask the manager.  If they still don't know, call around and find another title office that is familiar with the process..

would i still have to put the lights, blinkers, etc on it?

When you sign the affidavit you are affirming you have these and a couple other things.  There is no inspection to confirm your bike has them.  If you ride without any of these, you run the risk.  How much of a risk depends on how much road, which roads and which items you are missing.

YES!!!! You need to add those items, we are fortunate to have the opportunity to convert our dirt bikes to street titles here in Ohio, many other states are not able to do this, make your bike confirm to these minimal requirements so as to not ruin a good thing. Once you sign the form you are legally responsible for what you have stated on it.
