This is what I did. Whatever way you get it out, make sure to replace the bearing as well (18.)

(Edit: this next part is if you weren't replacing the case) 

Once you have it all back together, a dab of quicksteel or JBweld is more than efficient to seal the hole. The hole and JB weld are hidden from plain view.

I know this isn't the ideal solution but after 50 hours it has held up great.

I don't remember exactly what I did there but you could try leveraging or prying from the bottom side up through where 16, 17, 18 are. If they don't come out easily and you have to use a little force, make sure you replace the bearings.
