you can get 'X' amount of teeth onto a 2" diameter gear or a 2.5" diameter gear - it just depends on your tooth profile.

Also remember that a gear ratio of 2:1 can be worked into the following tooth combinations: 12/24,18/36, 24/48 and any combination of teeth like that, it just means the smaller gear turns 2 rotations while the larger gear only turns 1 and you can use whatever amount of teeth on each gear to make it work.

They would just design the two meshing gears to fit the space provided - they know the ratio they want, the approximate tooth profile required and work the design from there, it doesn't matter if your shaft spacing is 3" on center or 6" on center.
