Tire Changing Question

I'm trying to become good at changing tires. The devil is in the details. I got it down okay, though I'm pretty good at pinching tubes. One question that I can't find the answer to:

Looking at the detail of the valve stem of a Tr-4 valve. There are three items fixed to the valve. One is is a metal washer/spacer/grommet type of thing, then there are two nuts. Question #1: Do all three parts attach AFTER the valve stem is pushed through the rim. Or is the metal grommet placed directly on top of the tube under the rim?

Question #2: Why two nuts?

Question #3: I'm I the worlds biggest idiot for having to ask this question?

Washer and 1st nut stay in place. The second nut is removed before instal and replaced after the tube is installed. Many people omit the second nut or run it up against the valve cap so you can see if the stem creeps in use. The second nut is handy to retain the stem in place in the process of levering the tire on.
